Instruction: This form is to be completed by team leader(s) or coaches.  Please fill out the form and save its content.  Send your completed Basketball Tournament Team Roster Form (BBTTR-Form) along with all players photographs in a zip file to ersfna@gmail.com  no later than the stated deadlines outlined in the Registration Guideline Document. Please ensure all photographs match the requirement outlined in the Registration Guideline document.  ERSFNA reserves the right within the confines of the organizational bylaws to reject or accept a player.
