The subject heading of the e-mail should read, [Insert tournament year] "Youth Soccer Roster Form Team" [insert full team or club name].
All stated date of birth are subject to verification in accordance with the guidelines provided in the 'Registration Guide' document. Electronic submission of the form via e-mail will serve as signature. ERSFNA reserves the right within the confines of the organizational bylaws to reject or accept a player.
