Instruction: This form is to be completed by team leader(s), coaches or coordinators.  Please fill out the form and save its content.  Send your completed Youth Soccer Tournament Team Roster Form (YSTTR-Form) as an attachment to ersfna@gmail.com no later than the stated deadline. The subject heading of the e-mail should read, [Insert tournament year] "Youth Soccer Roster Form Team" [insert full team or club name].

All stated date of birth are subject to verification in accordance with the guidelines provided in the 'Registration Guide' document. Electronic submission of the form via e-mail will serve as signature. ERSFNA reserves the right within the confines of the organizational bylaws to reject or accept a player.
