This Complaint Form must be used when filing a sports related complaint with the ERSFNA involving an alleged denail of an opportunity to compete. The form must be completed the same day of the match for which it is filed. It shall be submitted to the ERSFNA's General Secretary no later than the specified time in the 'Coach's Handbook' along with the appropriate filing fee.
The complaint must be signed by the Claimant (or Claimant's authorized representative) or where a complaint is being brought on behalf of a team, by a representative of the team. If this complaint is being submitted electronically, the person sending the e-mail shall have been deemed to have signed the complaint.
In the event the dispute is upheld the filing fee will be refunded; however, if is the protest is denied the filing fee will NOT be refunded. All Executive Committee decisions are final.
